Home Care Nurse Job Description Job Title: Home Care Nurse (RN) Clinical Coordinator

Job Summary:
A Home Care Nurse provides professional nursing services to patients in their homes. The primary goal is to help patients recover, manage chronic illnesses, and maintain independent living through personalized, in-home care. This role requires a compassionate, autonomous, and highly skilled individual capable of providing high-quality care with minimal supervision.
Duties and Responsibilities:
Patient Assessment and Care Planning
- Conduct initial and ongoing comprehensive assessments of the patient's physical and psychosocial status.
- Develop, implement, and revise individualized care plans in collaboration with the patient, family, and multidisciplinary healthcare team.
- Monitor patient health status, vital signs, and symptoms.
Patient and Family Education
- Instruct patients and their families on their specific medical condition, treatment plans, and proper self-care techniques.
- Educate caregivers and family members on proper home care procedures to promote patient independence and well-being.
- Identify and suggest tools or devices that can improve the patient's quality of life.
Coordination and Communication
- Serve as the primary liaison between the patient and other members of the healthcare team.
- Document all patient visits, treatments, and communications accurately and in a timely manner within the electronic health record.
- Coordinate with community resources to provide additional support services as needed.
Required Qualifications and Skills:
- Education:
- Registered Nurse (RN) with an active and unrestricted state license (Compact license is ideal as we work in WV, MD and VA).
- Associate's degree in Nursing (ADN) is required; Bachelor of Science in Nursing (BSN) is often preferred.
- Experience:
- Minimum of two years of clinical nursing experience, preferably in a home health, hospice, or acute care setting.
- Skills:
- Strong clinical and critical thinking skills for independent decision-making.
- Exceptional communication and interpersonal skills.
- The ability to work autonomously and manage a caseload of clients throughout our service area.
- Knowledge of documentation standards and electronic health records.
- Valid driver's license and reliable transportation for home visits and clear motor vehicle report.

Working Conditions:
- This role requires travel to patient homes in various residential settings.
- The position may involve exposure to various home environments and requires physical stamina for lifting, moving, and assisting patients.
- The schedule may include rotating shifts, on-call duty, and occasional evenings or weekends, depending on patient needs or to supervise CarePros.
- Pay range is $30-35/hour
