Job Description

UPMC Hamot Transplant Services is looking forward to hiring a Full-Time Clinical Transport Coordinator I to join their team of Life Changers!

This position will work Monday through Friday, 8:00am - 4:30 pm with paid on call.


This role helps to support the through the transplant patient and family phase(s) providing teaching about medications, testing, labs, transplant process that a patient goes through to obtain and maintain a kidney transplant.


Title and Salary will be determined based upon education and experience.

Purpose: The Clinical Transplant/VAD Coordinator I is responsible for delivering nursing care using the nursing process. Working collaboratively with the transplant/VAD team and other healthcare professionals, they apply clinical knowledge and skills to ensure quality outcomes. This role requires entry-level competency in providing age-appropriate care, understanding growth and development across the lifespan, and interpreting patient data to meet individual needs.


Responsibilities:

  • Identify appropriate transplant/VAD candidates and interact with hospital staff.
  • Determine evaluation requirements and coordinate with multidisciplinary team.
  • Advocate for candidates considering developmental, educational, and cultural factors.
  • Provide data to regulatory agencies and ensure UNOS compliance.
  • Develop and implement teaching plans for candidates; maintain communication with patient and referring physician.
  • Monitor and document candidate status; update UNOS listing as needed.
  • Participate in peri-operative process per protocol; review recipient eligibility; facilitate admission/transfer.
  • Communicate with organ procurement agency; collect data for organ acceptance/rejection decisions; remove recipient from list per UNOS rules.
  • Monitor post-transplant/VAD patient progress (inpatient and outpatient).
  • Educate recipients and families on care responsibilities; reinforce follow-up and compliance.
  • Collect and review health and organ function data; maintain comprehensive documentation.
  • Implement physician-ordered drug therapy changes; arrange consultations and procedures.
  • Maintain communication with referring physicians and healthcare providers.
  • Assess and support medication and regimen compliance in collaboration with team.
  • Obtain ABTC certification; attend conferences; participate in education and outreach.
  • Accept feedback; identify learning needs; take action for professional development.
  • Complete self-review; adhere to on-call schedule.
Qualifications:
  • 2 years recent clinical nursing experience.
  • Demonstrated competence in professional nursing practice.
  • Diploma or Associates Degree in Nursing. BSN preferred.
  • Must obtain a Solid/Strong/Good or higher on annual performance review to be promoted to Clinical Transplant/VAD Coordinator II.
    Licensure, Certifications, and Clearances:
  • Registered Nurse (RN)
  • Act 34
  • It is preferred that within 2 years employee will obtain and maintain certification from the American Board for Transplant Certification (ABTC) as a Certified Clinical Transplant Coordinator (CCTC), CHFN (Certified Heart Failure Nurse), or other UPMC approved certification associated with the service line upon eligibility in order to be promoted to Clinical Transplant Coordinator II.

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
